Nurses push for a real voice in hospital AI decisions

Nurses are pressing hospitals and unions for a bigger role in how clinical AI is deployed.

Nurses, the largest group in the health care workforce, are demanding more say in how hospitals buy, build, and run clinical AI as the technology spreads deeper into patient care.

The pushback is highly visible. Bronx nurses at Montefiore who lost jobs blame administrative AI that they say took over their duties. Kaiser Permanente nurses in California have also walked off the job, with AI surveillance a central grievance.

The campaign is centered in the union. National Nurses United represents over 200,000 nurses, and its members staff both Montefiore and Kaiser. Contract negotiations are where AI demands now surface, according to STAT’s August 11 report.

Outside the picket lines, researchers make a similar case. Nurses interact with clinical AI more than most clinicians, yet their opinions rarely shape how tools get chosen or rolled out. A nursing school dean told STAT that nurse perspectives get overlooked, and advocates say earlier involvement would improve safety because nurses notice when tools fail and create extra work.

Hospitals are responding unevenly. Some are creating nursing seats on AI governance committees, while others treat AI procurement as an IT and physician matter. The strikes and layoff protests suggest the issue is moving from the break room to the bargaining table, with patient safety as the rallying point.
