AMA’s 2027 code set lifts AI billing codes to 43

The American Medical Association added ten AI-related codes for 2027, even as CMS and patient groups challenge its grip on medical coding.

Ten new billing codes land in the 2027 physician code set. They cover AI services that assist clinicians. The American Medical Association’s annual update takes effect January 1 and lifts the total number of AI-related codes to 43, from 33.

Those codes describe tools that support or assist clinicians rather than replace them.

Across the whole set there are 453 editorial changes: 299 additions, 74 revisions and 80 deletions. Other revisions cover ventricular assist devices, hernia repair, prostate biopsy, biofeedback, radiology and sleep medicine. The AMA also replaced its global maternity care bundle with a framework spanning antepartum to postpartum care.

The release lands in a difficult moment for the association. Patient advocacy groups including PatientsRightsAdvocate.org are suing to invalidate the AMA’s copyright over the numbering system, arguing that incorporation into federal and state rules makes it public property. CMS has separately sought feedback on alternatives, questioning a six-decade reliance on a private body with financial stakes in payment valuations.

Critics point to licensing costs, administrative complexity and a set that now exceeds 7,800 entries. AMA President Dr. Willie Underwood III defended the arrangement, saying the country needs a reliable way to describe physician care and calling the numbers the backbone of health data interoperability.

For smaller vendors without legal teams to lobby for custom codes, a published number is easier to price, sell and audit.
