Rural hospitals doubt AI will cut costs as Medicaid shrinks

STAT interviews with rural health leaders find AI delivering relief on documentation and burnout, but not the savings federal officials promise.

Rural providers facing nearly $1T in Medicaid cuts over the coming decade are being told AI will rescue them. Many of them do not buy it.

Trump administration officials have promoted the technology as a deflationary force in health care, funded in part by the $50B rural health transformation fund packaged with the cuts. CMS administrator Mehmet Oz has pointed to “AI-based avatars” as one answer for rural communities.

STAT interviewed dozens of rural providers, system leaders and health AI experts for the fourth installment of its Unraveled series. The recurring answer: the tools help, but the savings are unproven and the upfront cost is hardest for the systems with the least money.

Lori Dwyer, chief executive of Penobscot Community Health Care in Maine, has deployed ambient scribes. Documentation time fell, communication improved and clinicians got relief from administrative load. The technology did not lower costs, she said.

MaineHealth’s Trampas Hutches is accelerating adoption anyway, calling AI a coming “care team member” and a necessity for meeting demand.

Some leaders compared the vendor market to the dot-com bubble, unsure which products are worth buying. James McHugh of Berkeley Research Group was blunter: “There’s a lot of garbage out there.” He said AI adoption rarely delivers the promised labor savings.

Data infrastructure is another barrier. Faxed and handwritten forms, weak broadband and thin IT teams limit what models can ingest.

Aidoc chief medical officer Jesse Ehrenfeld reported real growth in remote areas. But the concern voiced across the interviews is that gains will concentrate in well-funded systems, widening the gap AI was meant to close.
