FDA advisers weigh the first blood test for many cancers

FDA advisers gathered Wednesday to weigh Galleri, which would be the first blood screening test cleared for multiple cancers at once. Grail presented its case, agency scientists presented theirs, and members of the public spoke before the vote. Recommendations from such panels usually guide the agency, but they are not binding.

The test has been on the US market since mid-2021 as a laboratory-developed product priced at $949. Grail says it picks up signals for as many as 50 cancer types, including many that no routine screen currently covers. The company frames it as a supplement to colonoscopies, mammograms and similar checks rather than a substitute for them.

Reviewers asked three questions: whether there is reasonable assurance the test is safe, whether it is effective for the proposed indication, and whether benefits outweigh risks. Panellists also weighed the need for post-approval studies.

Financial stakes rose sharply after Congress authorized Medicare to cover multi-cancer detection tests cleared by the agency. FDA staff briefing documents released this week flagged no major concerns, and Grail shares jumped more than 30%.

A companion randomized trial missed its primary endpoint, so the panel’s read on clinical utility carries unusual weight. Hundreds of companies are chasing blood-based early detection, making Galleri a bellwether for the whole field.

Approval would not settle the evidence debate, but it would create the first regulatory and reimbursement path other developers could follow.
