Fractyl’s one-time gut procedure cuts weight regain after GLP-1s

A single endoscopic treatment kept 80% of lost weight in place six months after patients stopped tirzepatide, against 67% with a sham.

The hardest problem in obesity medicine may soon be what happens once the prescription stops. Clinicians managing weight regain have had little beyond indefinite drug therapy to offer; Fractyl Health reported on October 16 that its Revita procedure narrowed that rebound in a randomized, sham-controlled pivotal cohort.

How the procedure worksWhat is still pending

Numbers from the six-month readout: 5.1% mean weight regain for patients who got Revita, 8.2% for those who got the sham, a gap with a p-value of 0.0023. Roughly 80% of the tirzepatide weight loss stayed off in the treatment arm, compared with 67% in the sham arm.

How the procedure works

Duodenal mucosal resurfacing is what Revita does. A clinician burns away the damaged upper small intestine lining, which then regrows, an effort to restore nutrient sensing and gut-brain signaling that chronic metabolic disease is believed to impair. One outpatient session covers it.

More than 30 US sites took part in REMAIN-1, which enrolled 308 adults with BMI between 30 and 45 who had never used a GLP-1 drug. All were put on tirzepatide until they had shed at least 15% of body weight, then taken off it and assigned 2-to-1 to the procedure or a sham endoscopy.

The six-month comparison rested on 176 Revita patients and 74 sham patients.

What is still pending

One co-primary endpoint has not been reported: how many Revita patients hold at least 5% weight loss at one year, assessed against an FDA performance goal of more than 50%.

Regulators have granted Revita Breakthrough Device status specifically for keeping weight off in obesity patients who stop GLP-1 drugs. A De Novo submission is the path Fractyl has signaled next. Post hoc results also pointed to longer ablations producing bigger effects.

Should the year-one endpoint hold, a procedural option for the field’s discontinuation problem becomes plausible.
