Perioperative nurses group issues AI safety guidance for operating rooms

The Association of periOperative Registered Nurses released new guidance July 22 urging health systems to govern, validate, and monitor AI tools continuously in operating rooms rather than treating them like standard technology rollouts.

As AI moves deeper into surgical settings, tools that predict surgical risk, issue intraoperative alerts, and analyze postoperative outcomes bring new risks. The AORN guidance cautions that these systems can introduce bias, drift in performance over time, and influence clinical decisions in ways that are difficult to detect.

“Hospitals cannot treat AI as just another technology implementation,” the organization stated. The guidance calls for keeping clinicians accountable for patient care even when AI tools are in use, establishing validation frameworks before deployment, and monitoring for performance drift after implementation.

The perioperative guidance is part of a broader push within healthcare to establish consistent governance for AI tools that affect direct patient care. Other organizations, including the Joint Commission and the FDA, have also released frameworks this year for clinical AI oversight.
