A four-tool AI suite at Singapore’s National University Health System could reclaim up to 850 pharmacist hours every week once fully deployed.

The NUHS Cluster AI in Pharmacy platform, or NCAIP, pairs medication reconciliation with triage and verification. Admission and Discharge MedRecon compare medication lists across care settings and flag relevant changes, MedTriage identifies patients who need pharmacist counseling, and MedVerify screens orders for dosing, contraindications and drug interactions.

In validation studies, the reconciliation tools posted accuracy rates above 88 and 90 percent, MedTriage reached 100 percent in spotting patients who required in-person counseling, and MedVerify exceeded 95 percent. NUHS stresses the tools generate recommendations while pharmacists keep final clinical authority.

Why it matters: NUHS pharmacy services support about 1.2 million residents in western Singapore, and the cluster’s pharmacists currently spend more than 350 hours a week reconciling medications, about 190 hours on routine triage for low-complexity prescriptions and roughly 310 hours verifying inpatient medication orders. NUHS estimates the full suite could save up to S$2M ($1.5M) annually.

MedTriage, which has secured funding and is targeted for rollout by the end of 2027, is expected to save up to 94 manhours weekly and cut in-person dispensing by as much as 20 percent. MedVerify and MedRecon await government funding and could deploy progressively from early 2028.

Built on NUHS’s ENDEAVOUR AI data platform, NCAIP took a team of 43 people six months and nearly 1,600 reviewed cases to validate. The project team has briefed pharmacy leaders at other public health institutions in Singapore as it explores a potential nationwide rollout.
