Remote care platform HealthSnap banks $25M to widen AI reach

The Miami chronic care platform lands a $25M growth facility to scale its AI-powered remote monitoring.

HealthSnap, a Miami provider of AI software for remote patient monitoring and chronic care, has landed a $25M senior secured growth financing facility led by Eastward Capital Partners.

The loan will refinance existing debt, shore up the balance sheet and speed up AI development, the company said. It also supports commercial expansion of its Advanced Primary Care Management platform across health systems.

HealthSnap works with more than 80 EHR systems and pairs its software with medical devices that beam patient data to clinicians automatically. AI care coordination identifies high-risk patients, and the platform automates documentation and reimbursement eligibility checks while feeding population health analytics.

The financing lets the company push real-world AI use across virtual care management, from decision support to patient engagement, CEO and cofounder Samson Magid said.

The company raised a $25M Series B in 2024 led by Sands Capital, bringing total funding to $48.5M. Customers include UnityPoint Health, Prisma Health, Mount Sinai Medical Center in Miami, Sentara Health and University Hospitals.

HealthSnap positions the round as fuel for chronic disease management, a category where providers are chasing both better outcomes and new revenue. Its pitch to health systems is that AI amplifies clinicians rather than replacing them, and the new capital is meant to prove that model at larger scale.
