Brellium has closed a $23M funding round and launched CortexLM, a platform that reads every clinical note before a claim is submitted rather than auditing a sample after the fact. Left Lane Capital and First Round Capital led the round, with Digital Health Venture Partners joining.
The logic behind the approach is straightforward arithmetic. Payers now audit far more aggressively, and by the time a documentation gap surfaces it has already become a denial, a clawback, or a care error. CortexLM checks each patient note against specialty guidelines, individual commercial payer policies, CMS billing rules, and quality reporting criteria while the encounter is still fresh.
The commercial hook is unusually direct. When a chart reviewed and cleared by Brellium later faces an audit from CMS, a Medicare Administrative Contractor, or a commercial health plan that results in a penalty, Brellium covers the cost instead of the provider. That shifts the liability that has kept many groups from trusting automated compliance tools.
More than 215,000 clinicians rely on the platform across 200 provider groups spanning all 50 states, Brellium says. The company reports it has audited over 22 million patient visits and protected nearly $2B in revenue. Users include Gentiva and Maxim Healthcare on the home health and post-acute side, along with Headspace and Rula in behavioral health. At Headspace, chart review coverage moved from 2% of therapy notes to 100%, with documentation compliance reaching 99.2%. The funding will push Brellium into additional specialties and deeper into the revenue cycle.
